Clay is returning 20-30% wrong domain names. Any clue why that might be happening?

Try Claygent and ask it to validate the result against any other data points you have on the agency

Suraj S. I got the same result from the waterfall. I don't think this waterfall was designed for gov agencies. Can you share what other data you have? For example, Carmel Police Department could be Carmel, California or Carmel, Indiana, so the city, state would help a lot.
