Issues with Create SFDC Record Functionality in Table Rows

My Create SFDC record column is only working on about 75% of my rows in this table - https://app.clay.com/workspaces/288936/workbooks/wb_Pgs8SmmSnx7A/tables/t_rnJmyrHrU6xN/views/gv_pKvCux4FWDUJ Any help on why this could be running into this error?
