The issue you're experiencing is likely because the fields haven't been published in Webflow yet. Webflow's API won't expose custom fields until they've been given a value in a published Collection Item and the site has been republished.
To resolve this:
1. Create and publish at least one item in your Webflow collection that uses the Name and Slug fields
2. Republish your Webflow site
3. Try the integration again - the fields should now appear in the dropdown menu
Also, when you do get the fields working, remember that slugs must be in lowercase letters and separated by hyphens (-) rather than spaces or underscores.
